I need the cover, spine and back cover of my novel as a single image (for print publication through CreateSpace), as well as the cover alone as a separate image (for ebook publication at Amazon, Kobo, etc.). Both files need to be at least 300 DPI, in CMYK or RGB color, flattened to one layer, and sized at 100%, and centered both horizontally and vertically (these are the specs required by CreateSpace). When printed, the book will have a 6 x 9 trim size. The maximum for the cover size is 40MB. The font used in the text of the book is Century Schoolbook, but this does not need to be reflected in the cover font. The cover and back cover should have a 0.125" bleed on all four sides.
The book's spine needs to be 0.554" in width. The book's title, Certain Freedoms, needs to appear on the spine, with 0.0625" of space on either side (including the tops and bottoms of letters such as y, g, or d where the letter extends below the baseline or above the mean line of the font).
I need the press's logo to appear, either vertically or horizontally, at the bottom of the book's spine. I have attached a copy of the logo.
The Back Cover:
Please keep the bottom right-hand corner relatively open for the placement of the book's barcode, which will be added by the printer. The back cover needs to have the following blurb appear, centered, in the middle of the cover (the first four words of the blurb -- "Russia, 1993. Certain Freedoms" -- need to appear in italics:

The cover should have a 0.125" bleed on all four sides. The cover 'in my head' centers on the close-up of a cracked and aged face from a Russian icon, preferably the face of the Madonna. Interposed on top of the face is a child's hand, reaching as though to touch the icon. The hand is small enough that it resembles a tear beneath the Madonna's eye.
This novel explores the spiritualism of Russian icons, but it is not an overtly religious book. I would love the cover to capture the artistic power of the icons, which in itself conveys a profoundly spiritual sense of awe. Having said all of this, the cover 'in my head' is not necessarily the best cover for the book. I'm open to new ideas.
